Before You Begin
Your account must be shared with your housing provider.
Please utilize the definitions of Assistance Animals under the Fair Housing Act (FHA) to determine which type of profile you need to create for your animal(s).
- Service Animals: Any dog individually trained (or in training) to do work or perform tasks for an individual with a disability. Other species are not recognized as service animals. The work must be directly related to the disability; emotional support is explicitly excluded as a service task.
- Support Animals: Trained or untrained animals that do work, perform tasks, provide assistance, and/or provide therapeutic emotional support for individuals with disabilities.
How to Create A Profile for a Service Animal
Step 1: Log In
You can start by logging into app.petscreening.com using your email address as your username.
Step 2: Select How Many Animals You Own
Once you're logged in, you'll be prompted to respond to the question “How many pets/animals do you have?”
You can increase the number by clicking the “+” button to the right of the counter.
Once you have selected the number of animals you have, you can proceed by clicking “Submit.”
Step 3: Select the Species
Select the species that most accurately represents your animal.
Please make sure to check the box that you are making a reasonable accommodation request, and follow the prompts.
Please click the Complete your Request button at the bottom of the screen to proceed.
If the reasonable accommodation box is not clickable and you are instead presented with information referencing your property’s landing page, please let us know so that we can further assist you.
Step 4: What Information Do I Need?
Animal Information and Photos
In this section, you will tell us the basics about your pet. You will need to share your pet’s:
- Name and breed
- Gender
- Spay/Neuter status
- Their birthdate
- Date of ownership
- How much they weigh
- The color (and pattern, if applicable) of their coat
- If they have a microchip (this is not required)
- Photos of your pet
Insurance
This section asks if you have insurance for your pet or your home.
- PetScreening does not require insurance.
- To skip this section, please click "No".
- Please note: Once you submit your profile, you cannot change this part. But don’t worry—it won’t cause any problems with your home or your lease!
Vaccinations
This section asks about the vaccinations your pet has received from the vet.

What vaccinations are required?
- Typically, the only required vaccination is Rabies. Some housing providers and municipalities may require certain vaccinations. Please check with your housing provider to confirm, and of course, follow your veterinarian's suggestions regarding pet and animal care.

What do I need?
- We ask that you upload a copy of your pet’s vaccination document from your veterinarian and enter the dates and longevity of each vaccination.
- Please note, we ask that you complete the vaccination details section, as the document only serves as support for the manually entered dates.

What if my pet has a medical exemption?
- We understand that animals may not have received vaccinations due to a number of reasons, including age, health, species, or otherwise. We ask that you provide documentation from your pet’s veterinarian.
Service Animal Details
You will be asked 4 questions:
- Have you read and acknowledged the assistance animal fraud statement?
- What state are you moving to?
- Is the animal required because of disability?
- What is the disability-related task the animal has been trained to perform?
- In other words, what specific ACTION is your animal trained to do that helps you?
Please note : PetScreening does not ask you to disclose your particular disability as part of your accommodation request.
Affirmations
The affirmations section is the final section of your profile prior to payment. You will be asked a series of questions regarding your pet and agreement to terms and conditions set by your housing provider. All affirmations will need to be answered prior to submitting your profile!
Step 5: Submit for Review
Once you're finished filling out the profile, click the “Submit for Review” button to make your request.
You should receive an email that the profile is in the "Review Status" once the profile is submitted or re-submitted!
